Statement of Income

You have two options when reporting your monthly earnings and employment-related child-care expenses:

  1. Enter your payroll deductions and child-care expenses on the form without submitting your paystubs and receipts.

    or

  2. Sign the bottom of the form and attach your paystubs and receipts.

If the form is not received on time, or if your caseworker cannot confirm the information, your case may be suspended until the information is provided.

Changes Report

Changes to your personal circumstances can be entered on the Changes Report form, which is now included in the monthly mailout.

This form gives you space to enter any changes to

  • where you’re living
  • your housing costs
  • your family members’ information.

You can also report changes to your caseworker in person or by phone.
