When something changes in your life, it could affect whether you are eligible for ODSP or the amount of income support you receive. You have to let your caseworker know.
That’s why at the end of every month, we send you a Changes Report form. You get this with your ODSP cheque or your direct bank deposit statement.
The Changes Report form and the Employment/Training Income Report form are two different forms. You need to use the Employment/Training Income Report form to report how much money you and the adults in your family make from a job or training program. You need to fill out the Changes Report form so that we can provide you with the right amount of income support.
No. Even though you receive the Changes Report form every month, you only have to fill it out and send it in when you have new information to tell us about.
For example, you need to let us know if you:
You and each family member can receive up to a total of $6,000 in a 12-month period from gifts, trusts, life insurance policies, and other voluntary payments without impacting your income support. |

After you fill out the form:
If you forget to fill out the Changes Report form, you can still contact your caseworker directly and tell them about your changes.
If you don't report your changes, you may not receive the right amount of income support. If this happens, you will have to pay back any money that you weren't supposed to receive.
