When something changes in your life, it could affect whether you are eligible for ODSP or the amount of income support you receive. You have to let your caseworker know.

That’s why at the end of every month, we send you a Changes Report form. You get this with your ODSP cheque or your direct bank deposit statement.

Why do I need to fill out the Changes Report form?

You need to fill out the Changes Report form so that we can provide you with the right amount of income support.

Do I need to fill out the Changes Report form every month?

No. Even though you receive the Changes Report form every month, you only have to fill it out and send it in when you have new information to tell us about.

What kind of changes do I need to report?

For example, you need to let us know if you:

• start or stop getting money, or get a different amount of money, from a source that is not from a job or a paid training program. These sources could include:
• friends or family members
• the Canada Pension Plan Disability benefit (CPP-D)
• the Workplace Safety and Insurance Board (WSIB)
• Old Age Security, and
• child support
• buy, receive, sell or give away any assets, such as investments, property or vehicles
• need to pay more or less for your shelter, such as rent, hydro or heating
• have changes in your living arrangements, such as people moving into or out of your home
• change your address
• change your phone number
• have changes to the size of your family, such as the birth of a child
• have been admitted to a hospital or institution, and
• will be leaving Ontario for more than 30 days.

Do I use the Changes Report form to tell you about changes in my work earnings?

No.

You need to use the Employment/Training Income Report form to report how much money you and the adults in your family make from a job or training program.

What do I do after I fill out the form?

After you fill out the form:

• sign and date it
• attach copies of documents or receipts (keep the originals) so that we can make sure your new information is correct (for example, a new rental agreement if your rent changes, or a birth certificate if you have a baby), and
• hand deliver, mail or fax the form to your local ODSP office as soon as possible, but no later than the 7th day of the following month. For example, if you have a baby in September, you must let us know by October 7th at the latest.

What if I don’t fill out the Changes Report form?

If you forget to fill out the Changes Report form, you can still contact your caseworker directly and tell them about your changes.

If you don't report your changes, you may not receive the right amount of income support. If this happens, you will have to pay back any money that you weren't supposed to receive.
