Completing, saving and printing HTML Forms
HTML forms are available for people who use screen readers or other accessibility equipment.
Completing the forms
If you are applying for ODSP Employment Supports, you can fill out the application package on your computer screen and then print them once you have completed them.
Before you start, you may wish to read the instructions on how to complete and submit your application.
When you have completed your applications forms, you must sign the forms and submit them to your local ODSP Employment Supports office by mail, in person, or by fax.
Here’s how to fill-in the HTML forms.
- Click on the form title to open the form.
- Click on the first answer space and begin typing your information.
- Use the TAB key on your keyboard to move to the next box. You can use the SHIFT/TAB to move back.
- You can also use your cursor to move from field to field.
- Click your mouse or press the ENTER key to fill-in "checkboxes".
- After you have completed the form and have reviewed the information to make sure it is correct, you can print it. You may also want to print each page as you complete it so you don't lose your information.
Saving the forms
Important: The HTML version of forms will not allow you to save an electronic copy of a completed form that contains the information that you entered. You should always print a copy of the completed form before closing the file.
Please remember that you are responsible for the completeness and accuracy of all the information you provide on the forms.
Printing the forms
Use the Print function under "File" in the drop down menu at the top of your browser to print out the HTML forms.
