There are three steps to applying for Ontario Disability Support Program Employment Supports.

Step One

Complete the Employment Supports Application Package.

Where to get the application package

• The application package is posted on this website. You can print off a copy of the package and fill out the forms.

  or

• You can visit your local Ontario Disability Support Program Employment Supports office and pick up the application package. 

  or

• You can call your local Ontario Disability Support Program Employment Supports office and ask them to send you the application package.
  
  Make sure you fill out all the parts of the application. A friend or a family member can help you. You can also ask someone at your local Ontario Disability Support Program Employment Supports office to help.

Step Two:

In some cases, you may need to verify your disability.

• Get the Verification of Disability/Impairment form. It is in your application package.
• Sign the Consent to Release Medical Information page.
• Take the form to your doctor to fill out.

Step Three:

• Send your completed application package to your local Ontario Disability Support Program Employment Supports office.
  
  Staff will review your application and tell you what your next steps are.
