This guide was developed by adult developmental services providers in Toronto and York Region. It describes the transition planning process, including: best practices, steps to follow, things to consider when planning, and the roles and responsibilities of families and schools. Tips, tools and other resources are also provided to help in the creation a transition plan for a child who has a developmental disability.

National Association for the Dually Diagnosed (NADD) Ontario - Habilitative Mental Health Resource Network

The Habilitative Mental Health Resource Network is the Ontario Chapter of the National Association for the Dually Diagnosed. The National Association for the Dually Diagnosed (NADD) is located in New York, with membership from across North America, Europe, and Australia. NADD has an important role in disseminating information and stimulating interest in clinical, programme, and policy issues related to dual diagnosis.
The website offers a wide range of information related to dual diagnosis (the coexistence of a developmental disability and a mental health problem) and can be used to find out about and link to other organizations/services related to dual diagnosis, contact local dual diagnosis committees across Ontario, and to keep current regarding dual diagnosis training.
